Cannot place action buttons....!

I cant put action buttons in the main bar, it efin gettinme heated.

For some reason this bugs out sometimes and you have to hold down Shift to place an icon on the action bar. After that additional actions usually will place down fine.

I have had same problem and used same solution as Gryphon mentioned.
